【摘要】目的：分析探讨临床常用雌孕激素制剂的药学特点。方法：举例分析了雌二醇、黄体酮、甲羟孕酮等雌孕激素制剂的药学特点，加强用药监测，保证用药安全。结论：分析雌孕激素制剂也可以从剂型、药物代谢动力学、不良反应和用药监测几方面着入分析，从而提高患者用药安全性。
【关键词】雌孕激素制剂；临床常用；药学特点
从近些年来看，雌激素水平低下女性和绝经期女性的治疗受到越来越多的关注，治疗这些女性所用使用的雌孕激素药物也逐渐成为了关注的重点，本文举例分析了雌二醇、黄体酮、甲羟孕酮等雌孕激素制剂的药学特点，旨求提高女性的生活质量，现分析如下：
1、雌二醇
雌二醇是一种天然雌激素，它和雌酮、雌三醇都可以归入为天然雌激素的行列中。从天然雌激素來看，其生活活性比较高，故以关于此种激素为先导物的结构改造，改造的重点不是为了更大提高活性，而是为了提高药物的作用时间，使药物的作用效果更长，也是为了患者使用药物方便。
从雌二醇的剂型来看，它的给药途径有阴道、口服和注射、以及经皮给药等多种途径，从阴道给药的方式来看，雌二醇剂型有阴道环、阴道片和乳膏等，从经皮给药这种治疗方式来看，雌二醇有喷雾、贴片和软膏等剂型，多途径给药方式可以解决患者耐受问题。
从雌二醇的药物代谢动力学来看[2]，最容易被机体吸收的部位为皮肤、胃肠道，同时粘膜的药物吸收要过也很理想。从口服用药这种方式来看，口服一般会造成首过效应，其效应造成生物利用度降低，采取一定措施可以力高生物利用度，但是也只能提高4%作用[2]，故为了提高生物利用度，采取更换给药途径是最佳考虑。从雌二醇代谢动力学来看，此种药物会在人体的肝脏内由细胞色素P450发生酶代谢，从而代谢为雌酮硫酸化和葡萄糖苷酸集合物，以及代谢为雌三醇，且雌三醇的活性不高。从人体排泄来看，其能通过人体的肾脏，也可以通过人体胆汁进入到肝肠循环，经尿液或粪便排出，半衰期在1h左右。在对患者的治疗过程中，透析治疗可以消除一定量的雌二醇，故在透析治疗的雌二醇选用中，因注意其剂量；而肝功或胆汁淤积的患者，由于疾病因素会导致雌二醇代谢变低，也会影响到药物排泄，故在治疗的过程中严密监测患者的病情变化和用药剂量。
从雌二醇的用药不良反应来看，不良反应可涉及多个系统器官，如消化系统、生殖系统、循环系统和内分泌系统等。从生殖熊的不良反应来看，一般表现为月经紊乱或乳房压痛，从循环系统来看，多有水肿的现象发生，从消化道系统的不良反应来看，有恶心呕吐或胃胀等情况出现。如果患者长期大量使用雌二醇，那么还可能发货时呢过罕见的副反应，比如心肌梗死、视网膜静脉栓塞、子宫内膜癌等。因此，在使用雌二醇的治疗过程中，应该加强对患者的不良反应监测。
从不良反应监测来看，应定期检查血压、定期自检乳房或到医院定期检查、严密监测甲状腺功能、血浆甘油三酯和血钙水平，对于未成年人如果需要使用雌二醇，那么应该对未成年人的骨骺以及生殖器官的发育做出严密监测，对于有眼部疾病繁盛的患者，如部分视野损伤患者要及时到眼科进行检查，对于心脏病或是肾功不全的患者，那么要检查患者的体内是否有液体潴留的情况存在。
2、黄体酮的药学特点
黄体酮又被称作为孕酮，其是人体内天然存在的孕激素，人体排卵后，破裂的卵泡组织能够形成黄体，而黄体的分泌和合成作用，其可以形成黄体酮，这种孕激素可以让女性的月经和妊娠得到维持，可以抑制女性排卵，故有避孕作用。
从黄体酮的剂型来看，其给药途径在目前有三种，分别为注射、阴道给药和口服用药，从阴道用药上来看，其一般有片剂和凝胶等。
从此种药物的代谢动力学来看，口服的方式仍然会造成生物利用度变低，选择其他用药的方式可以避免首过效应，若要通过口服用药来提高生物利用度，可以采取微粉化处理、油性载体、进食时口服等方式，这些方式均能从一定程度上提高黄体酮的生物利用度。黄体酮蛋白结合率很高，其可以达到98.5%左右，激素蛋白、白蛋白是换体统结合最高的物质，从代谢来看，黄体酮可以通过肝脏大写，可以代谢为孕二醇、硫酸盐等，同样也能代谢成葡萄糖苷酸结合物。黄体酮的半衰期较短，通常为几分钟时间，主要通过肾脏由尿液排出。
黄体酮的用药不当，也可能造成患者多个器官系统等出现不良反应，例如在生殖系统中表现出的不良反应有会阴疼痛、乳房胀痛等，从消化系统表现出来的不良反应有恶心、便秘等，和雌二醇不同的是，黄体酮可能造成患者的免疫系统或神经系统出现不良反应，如头痛失眠、病毒感染等。故此，在使用黄体酮的治疗期间，应该严密监测患者的血脂、血压水平，应严密监测患者的葡萄糖耐受，如果是使用阴道给药，那么应在黄体酮使用6h之后再采取其他药物的阴道给药，不可联合使用其他阴道用药药物。
3、甲羟孕酮
从甲羟孕酮这种雌孕激素药物来看，其是在黄体酮上进行改造后的一种药物，主要是在黄体酮结构基础上进行了6位甲基取代，并且进行了17α位乙酰氧基取代，这些取代可以让药物的代谢时间得到延缓，那么可以从更大程度上提高脂溶性，提升药物活性。
甲羟孕酮的给药途径也有三种，一般为皮下注射、肌内注射或口服用药，从注射的理想位置来看，人体大腿上部或者腹部是比较理想的注射位置。从这种药物代谢动力学来看，运用口服用药的治疗方式，其生物利用度同样不高，运用肌内注射，其药物的半衰期明显延长，可以达50天左右。
总的来说，在使用雌孕激素制剂治疗时应注意剂量控制，对高血压、糖尿病或是高血脂患者应加强血糖、血脂、血压的监测，对于各种不良反应及时做检查，从而保证用药安全。
